Failure to Launch: How Severe a Problem?

Difficulties are common at the transition from dependent adolescence to independent adulthood - but in some cases they begin getting described as "failure to launch" or "hikikomori" or "NEET." How big a deal is this problem, really? And why does it seem to take so much effort to resolve, when all the doors are swinging wide open?
