LOCKING & UNLOCKING OF KNEE | Screw Home Mechanism # Coupled motions

Detailed lecture on the LOCKING and UNLOCKING mechanism of KNEE COMPLEX.The locking and unlocking of KNEE COMPLEX, is something that is always a challenge to understand.Here we have explained mechanism of locking and unlocking in most simplified manner.Locking and unlocking of knee is one among the coupled motions that takes place in knee joint.Its also known as SCREW HOME MECHANICS,AUTOMATIC ROTATION OR TERMINAL ROTATION.It is very important for the achievement of RANGE OF MOTION that is in particular full extension at knee joint.
During LOCKING that happens at last 30 degree of flexion to extension or in particular at last 5 degree of knee flexion to extension the smaller tibial lateral plateau stops it motion but the medial plateau continues its motion.It makes lateral plateau a pivot around which medial plateau moves and finally at last 5 degree the medial plateau rotates laterally along with extension. And fro flexion to happen again the medial plateau should rotate medially which is achieved due to articular surface peculiarities and POPLITEUS MUSCLE.
So LOCKING MEANS EXTENSION ( TERMINAL) combined with LATERAL ROTATION
and
UNLOCKING MEANS FLEXION( INITIAL) combined with MEDIAL ROTATION.
It happens in both weight bearing and non-weight bearing.The above happens in non weight bearing and in weight bearing the following happens:
LOCKING MEANS EXTENSION of femur ( TERMINAL) combined with MEDIAL ROTATION
and
UNLOCKING MEANS FLEXION of femur ( INITIAL) combined with LATERAL ROTATION.
