After a grizzly bear chased a worker at the San Francisco Zoo, longtime tensions over employee safety and animal welfare reignited.

A Chronicle investigation reveals several incidents in recent years in which employee safety or animal welfare at the San Francisco Zoo was compromised.

This situation has led many workers to resign or say they have lost faith in the management of the 95-year-old institution.
