MOST CHALLENGING Otomycosis Fungal Ear Infection Removal from Ear with Glue Ear - #370

This blocked ear wax removal extraction procedure is performed by UK pioneer and world-leading endoscopic ear wax removal specialist Mr Neel Raithatha (Consultant Audiologist) aka 'The Wax Whisperer’ at his clinic in Oadby, Leicester, UK.

Ear wax removal is necessary when ear wax build up becomes impacted. Symptoms causes by earwax blockage include: earache (otalgia), conductive hearing loss, blocked ear, vertigo, tinnitus, itchiness and irritation, occlusion etc.

The client attended with Otitis Externa and a fungal ear infection called Otomycosis. He has been suffering from chronic ear infections now for several months, this despite receiving antibiotic treatment from his GP. On his last visit, he also had a visible retraction of his eardrum due to Eustachian Tube Dysfunction.

His ear canal was very inflamed and narrow. It was difficult for me to even insert the Zoellner suction probe inside. There was little working space as a result. I successfully managed to remove the outer infected debris which was occluding the ear canal. This revealed his fungal infection. I managed to remove a large piece of the otomycosis, which had visible spores and ‘fibres’, almost resembling a spiders web. I questioned the client as to whether he had been using cotton buds or q-tips due to the fibre-like appearance, which he had not.

The was some remaining fungal infection which had adhered itself to the eardrum. This was extremely difficult to remove as it would not suction off, despite obtaining a good suction grip. His ear was also naturally very tender due to the inflammation, and I worried about causing him pain and discomfort. I was also worried about using any cerumenolytics (ear wax softening drops) for this reason too, as they may agitate the ear canal wall. I therefore attempted to peel off the surrounding skin, in the hope this was connected to the fungal infection and thus peel off too. However, it wasn’t to be the case.

I therefore applied some sodium bicarbonate ear wax drops for a few minute, softening the edges of the fungal infection, in order for me to peel. It was extremely difficult to do so, because as mentioned above, there was very little available space to manipulate the Fine End inside his ear canal.

After completely removing the debris and fungal infection, he still felt his ear to be blocked. I performed Tympanometry, which can be described as a pressure test to check how mobile the eardrum and middle-ear is. In the presence of Glue Ear, there would be minimal, if no compliance/mobility, of the eardrum, illustrated as a ‘flat’ Type B tympanogram (with normal ear canal volume), which was found to be the case. The client has been referred to his GP for treatment accordingly.
