When looking at a healthy spine, the sagittal or side view should not be straight. The neck, mid-back, and low-back are all supposed to have curves.

These normal curvatures are not like the ones for scoliosis, and when found in a healthy spine, they are called lordosis and kyphosis. These healthy curves work together to help the body with gravity, mechanical forces, and stress.

When these forward bending, "S" shaped curvatures appear on the cervical, thoracic and lumbar spine, we know the spine is working as it should and is healthy.

These healthy curves also have an expected range or ideal range. For cervical and lumbar lordosis, the ideal curvature is 40 degrees. But some patients might have healthy curves between 30-35, 40-45, and over 50 degrees still be their normal.

When these curves are outside of their traditional range, they are considered abnormal. Hypolordosis, for example, means that the spine has lost curvatures and is becoming straighter than normal.

The most common hypolordosis happens on the cervical spine. People are often diagnosed with a loss of lordosis on the neck, and sometimes these patients are referred to as "straight neck" as well. In some instances, patients are diagnosed with the opposite, which is a kyphosis of the neck. In this case, the neck bends over the other way.

Another common term for hypolordosis is "military neck" because the neck appears to be so straight. A hypolordosis in the neck directly affects how the nervous system functions, and that is why it is very bad when patients are diagnosed with it.

This can lead to malfunctions in the nerves and joints, making the cervical spine less efficient in dealing with gravitational forces. This makes patients more vulnerable to pain, such as headaches, pain running down their arms, and similar issues.

A hypolordosis in the low back is the same. It brings many issues for the patients. In this case, patients are now dealing with more compression forces, causing pressure in low back tissue, which leads primarily to pain.

Also, when the posture is overly straight in the mid-back, this is called "flatback syndrome."

We never want to see patients with hypolordosis because it will directly connect to how much degeneration will take the patient's spine in that area.

Patients are often diagnosed with "spinal arthritis," but this is not the correct term. What happens is spinal degeneration. This is because what we see in these patients is a localized degeneration in the area of the loss of lordosis.

The main takeaway is to understand your spine does need curvatures on the side. These natural curvatures are part of a normal spinal alignment.
