Presenting: Donald Hutchinson, Nurse Practitioner, North West LHIN
In this session, Donald will explain how early identification and using a team facilitates a palliative approach to care in the community for patients with congestive heart failure (CHF). Challenges in early identification and key issues in palliative management of CHF in the community will be discussed, and the importance of goals of care discussions reviewed.
