【Chris Chang Ortho】440. Space Maintenance for Mutilated Class III

Abstract

This true Class III case was treated non-surgically with upper low torque and lower high torque PSL brackets, bite turbos and Class III elastics. The anterior cross-bite was corrected within only 3 months. Closing space for Class III patients with a mutilated dentition was not advised, especially in the upper arch. NiTi open coil springs were used to maintain the space for future implant replacement. Closing space with sliding mechanics on SS rectangular wires was facilitated by balancing lingual and buccal forces to prevent binding of the archwire due to a molar mesial-in rotation.

本案例藉由非手術的掩飾性矯正治療方式處理第三類咬合不正，在上顎選擇low torque矯正器，下顎選擇high torque矯正器，並使用bite turbos墊開咬合，合併第三類橡皮筋來去除前牙錯咬的咬合干擾，讓前牙錯咬在短短三個月內即可改正。「缺牙」的第三類咬合空間，特別是上顎缺牙，並不適合把空間關起來。治療過程中可使用NiTi彈簧維持空間預備日後植牙贋復，下顎關閉大臼齒空間則適用內外對關來避免大臼齒旋轉之副作用。
