Abstract

Case 1
When we have a class II ,division I and large overbite patient, we have to consider about three important things. They are torque selection, bite turbo, and light short elastics. In this case, without extraction, upper is standard Q and lower is low Q braces. Because deep bite, bite turbo put near the gigival margin of the central incisors. The bite turbo resin should be a little overloaded, the indicator line on the gingival direction, and should gentaly put on the tooth surface. The elastics should be put on 3-5 or 4-6 and Quail (2 Oz) would be used.

Case 2
When we have a patient using Class II elastics but can not get class I occlusion for several treatment time. We have to focus on the occlusal interference, that will influence the elastic effects. First, we should remove the interference then we can get a better result with elastics.
In orthodontic treatment, if patients have cavities on the proximal areas, we always use the open coil spring to open the space for restore the tooth without destroying too much tooth structure.

Case 1
當我們遇到 class II ,division I 並且有大 overbite 的病人, 治療時必須要考量三件事. 那就是矯正器的 torque 選擇, bite turbo, 和 輕且短的elastics配戴. 這個病例裡, 採取不拔牙, 上顎矯正器選擇標準角度, 下顎選擇低角度. 因為病人是深咬, bite turbo 放在上顎兩顆正中門牙並且靠近牙齦. bite turbo 的樹脂要放的稍微比需要的多一點, 突出的指示線要對著牙齦面, 並且輕輕的放置在牙齒表面再照光. 橡皮筋要選擇 Quail (2 Oz) ,放在上顎犬齒到下顎第二小臼齒或上顎第一小臼齒到下顎第一大臼齒.

Case 2
當我們遇到病人配戴 Class II elastics 一段時間過後卻一直沒辦法更正成 class I 咬合. 這時候我們要注意是否有咬合干擾, 因為這會影響到橡皮筋的效果. 首先我們要先去除咬合干擾( 將上顎兩側犬齒往上移動 ), 這樣橡皮筋的佩戴才會有預期的效果.
矯正治療期間, 如果病人在牙齒鄰接面有蛀牙, 這時我們會用 open coil spring 把牙齒彈開再填補牙齒, 如此可以直接填補蛀牙, 減少破壞好的齒質.
