Abstract

Asymmetrical use of elastics is a common method in the correction of midline deviation where space is the key in facilitating tooth movement. In this case the patient’s lower midline is shifted to the right by 1.5mm with an overjet of 0mm, lacking the critical space for midline correction. An efficient alternative is to hang a left, midline-crossed elastics in a figure “Z”, which exerts a leftward and backward force to the mandible and creates a space needed for midline correction. Here are multiple ways to generate space for midline correction: first. increase overjet by anterior root torquing springs; second, avoid occlusal interference by wire bending; third, eliminate heavy occlusal contacts by handpiece, and finally perform interproximal reduction(IPR).

不對稱使用橡皮筋是常見改善中線不齊的方法之一，關鍵在於利用空間來促進牙齒移動。在本案例中患者的下中線偏右1.5mm，而前牙區的overjet為0mm，缺乏改正中線的移動空間。一個有效的替代方案為左側的跨中線Z字型掛法，將下顎同時往左側以及後側拉，來創造空間促進中線移動。獲得空間的方法有下列幾種：1. 利用anterior root torquing spring來增加overjet，2. 進行Wire-bending避開咬合干擾，3. 以hand piece修掉過重咬點，4. 做interproximal reduction(IPR)。
