Path of Exile - I dropped a Mirror of Kalandra!
I heard the sound and thought it was an exalted orb, when I read the text I just thought myself: the game can't crash now, this one can't stand on the floor :D
It took roughly 150 fully juiced 100% Delirium Maps to drop this Mirror, and I guess I was very lucky. So, indeed, the rarest item by far on Path of Exile.
