REQUIREMENTS FOR ADULTS
FRESH CASES: Adult (18 years and above)
1. Letter of identification from Local Government/State of origin or residence or National Identity Card
2. Completed passport application form and 2 passport sized photographs
3. Completed Guarantors’ form attached with Photocopy of Data page of Guarantor’s Nigerian E-Passport and one passport sized photograph of the Guarantor;
4. Marriage certificate, where applicable;
5. Birth Certificate (certificate issued by the National Population Commission shall be required from applicants whose date of birth is with effect from Dec 1992) or Age Declaration attached with one passport-sized photograph and duly endorsed by a Commissioner for Oaths
6. NIS Acknowledgement slip and evidence of payment
7. Letter of application
8. Admin fee (www.nigerianembassy.nu)
9. Copy of your resident permit/dual passport

RENEWAL CASES: ADULTS (18years & above)
Expired Validity
1. Completed passport application form
2. NIS Acknowledgement slip and evidence of payment
3. Application Letter;
4. Passport Booklet;
5. Photocopy of Data Page of Passport;
6. Admin fee (www.nigerianembassy.nu)
7. Copy of your resident permit/dual passport
Extension of Validity
1. If Validity is more than Six (6) Months, Applicant should state reason(s) in an application letter addressed to the Comptroller General, Nigeria Immigration Service, Abuja.
2. Completed passport application form

4. Passport Booklet;
5. Photocopy of Data Page of Passport;
