The Occlusion Test of the Heart Lung Machine: what it means!!! Understanding made easy. Part 4
The occlusion test mimics the action of blood in a "just occluded" state. and medium occlusion is said to have the least hemolysis rate. Roller pumps are basically occlusive pumps and by compressing a segment of the blood-filled PVC or any other tubing, blood is pushed ahead of the moving roller, thereby producing a continuous and rather nonpulsatile or laminar blood flow. The output of this pump is determined by the RPM of the pump, size of the tubing and the length of the track within the backplate. The hemolysis rate increases with increase in the RPM and ofcourse the number of rollers (1,2 or 3). This can be attributed to several factors, including mechanical compression, friction heat, and sheer stress.
If the pumps are under occlusive or lightly occlusive, then the patient will be hypoperfused during the CPB run and after termination of CPB, the arterial line, vent and sucker, if not clamped, will exsanguinate the patient completely. So, medium occlusion is most desirable in terms of forward output, hemolysis and tubing wear and tear. There are many ways of doing this test and one may refer to the text books of perfusion for further details..
Pump calibration (the relationship between the revolutions per minute of the pump head and the flow per minute generated by the pump) should remain relatively constant over a period of months and should be done once in 6 months.
