The natural history of diminutive, small and large colon polyps

There is robust evidence that almost all colon cancer evolves from benign polyps. The duration of transformation from benign to malignant is slow, probably a decade or more. Consequently, screening for, and removing benign polyps, reduces the incidence of colon cancer. Bowel screening is now embedded in most developed healthcare systems. Fecal immunochemical tests (FIT) screening is designed to detect occult bleeding and a positive test calls for a colonoscopy to seek and remove colon polyps. Some national programs skip the FIT step and recommend screening colonoscopy despite the invasive nature of the procedure and procedure related complications including bowel perforation. The established mantra is "see a polyp and excise".
