Zarzaur Law, P.A. TV: What is a TBI and How is it Sustained in a Car Accident?
Attorney Joe Zarzaur and Dr. Evan Malone discuss Traumatic Brain Injuries (TBI) and how they are sustained in a car accident.
Experts estimate that an average of 137 Americans dies of TBI-related complications every day.
When people hear TBI, they think, “Oh, my gosh, your skull must've been fractured and you must have had a bunch of blood and they must have to repair your, your fractured skull!.” TBIs happen on soccer fields, football fields, every day.
HOW DOES A TBI HAPPEN?
TBIs happen in car wrecks all the time. Even if you don't have a cut on your head, you didn't have an impact with something in the car, like the A pillar, or the B pillar, or windshield - they happened because your brain is floating.
The skull is the hard case that the brain lives in. Between the brain and the skull is a fluid layer and that's the cerebral spinal fluid. That fluid pads your brain in your day-to-day activities, but when you have a forceful impact or sudden deceleration or acceleration it’s not enough to stop the brain from impacting the skull or shearing along the irregularly shaped inner lining of the skull. Think of something floating inside of a harder shell or case, and you throw that thing up against the wall, the case is going to hit the wall, but the thing that's floating inside is going to keep going and it then impacts the shell. So, and when your brain is moved around jostled in a car wreck, even if it's not a significant catastrophic car wreck, you're still going to have really harsh sudden movements.
WHAT ARE THE COMMON SYMPTOMS OF A TBI?
- Unsteady on your feet
- Dizziness
- Difficulty with sleep, wake cycle
- Irritability
- Loss of actual sense, like loss of smell
- Sensitivity to light
- Hearing not as sharp
- Short term memory loss
CAUSES OF TBI
1. Falls - like in your home setting
2. Blow to the head - that includes your sport and recreation stuff (numbers have gone up in the last decade, but it's not because they're having it any more often it's because we are more aware of needing to diagnose it).
3. Car wrecks
WHEN DOES A CONCUSSION BECOME A TRAUMATIC BRAIN INJURY?
What typically happens and how is it diagnosed? (A TBI is not diagnosed with one single test or one single visit to a doctor's office).
You start with your primary care doctor or with even an ER and then you will be referred somewhere. You then end up seeing a specialist, neurologist, or neuropsychiatric for workups
A neurologist will look at your neurologic function, things you can feel touch, movements you can make things like that. The neuropsychiatric evaluation is how your brain processes information. So, it's kind of like doing like quizzing your brain. So instead of sitting down with a pen and paper and doing arithmetic or spelling, there are other processes that they test, like how sensible you are to different orientations, and they trick your brain and see if it can work around the problem, they're putting in front of you.
