Previously we were doing fresh cycle IVF for ART techniques we were short of certain things. We never had good medias to freeze the embryos which we get in a surplus . Like if you have got five embryos was there was no proper media to freeze them so we would either transfer it back into the uterus or transfer the best embryos into the uterus and discard the rest. But with the advancement in the IVF with so much of new things coming up in the IVF now we have a vitrification media which we can proudly say we can freeze this extra embryos which are formed during the IVF cycle. So now well previously we were doing fresh embryo transfers where the embryo after retriving after fertilization after it has grown up to blastomere stage we transfer it back into the uterus. But over the years we have found that this was not matching with the window of implantation like the window of implantation was going ahead or the embryoblast is going ahead these two were not matching. So there was lot of unsuccessful IVFs, failures in the IVFs and people were getting disappointed and depressed with the cycle. And sometimes what happens is when you stimulate a cycle the hormones are moved the e2s are high. So that also doesn't match for the implantation that is a defective it acts as a negative thing for implantation of the. So we have stopped doing fresh embryo transfers . Still we do it for a people who have one or two eggs or who have very few eggs when we know that the her hormones are not that high enough. So we still doing it but not for everyone now what we have accepted and we are doing is we are going more towards natural menstrual cycle we are depending on it. So we are doing frozen embryo transfers where we freeze whatever embryos good embryos we grade them up and then we freeze them up in a batch of two in each cryotop and we prepare her uterus. We see if her uterus is good or lining is good and lining is ready to receive the embryo we prepare her it can be done in a modified natural cycle or it can be done totally with the medically frozen cycle. So in natural cycle we have to do blood test number of times to match when she's getting ovulated when is a window of implantation when is a peak in the LH, when is a peak in the P4 and all these hormones. So it's like troubling the patient but for few younger ones we try to do a natural cycle. But for a person who is more than 35 we always go for planned frozen embryo cycle where we prepare our uterus by giving us some hormones and building up our endometrium and building up the blood supply and everything and when everything is ready and her progesterones are low we transfer it in the window period which commonly ranges by average from 18th to 21st or 22nd day of the cycle because when we calculate at the ovulation it usually happens from 13 or 14th day and then it takes another 5 days for the blast to form. So going through the natural cycle calculations we find the window period to be somewhere around 18th to 21st day. So we transfer in that period when everything and all the factors are good and not compromised resulting in a positive pregnancy.
