Baby Alexandra's story: 'a culture of not listening' led to tragedy

After 40 hours of labour, Alexandra Futák-Campbell was born using Kielland's forceps. She suffered a severe spinal injury and died three days later. Her mother Beatrix offers a powerful account of the aftermath, and shares her frustration of dealing with a health system that seemed unable to listen, or to learn from its mistakes.
