DIY - $50 seamless septic cover build (how to)

I got an $800 quote from wundercovers for a removable paver access cover, which was slightly over my budget. (... and by slightly I mean extremely.) My paver contractor paved over my septic access lid even though we had discussed it at length before that project started. Nonetheless, faced with the problem at hand, I kicked around dozens of ideas in my head. Here's what I came up with and how I built it. I think it turned out fantastic. The best part was that the concrete blocks for riser/base, the "D" rings and the construction adhesive added up to less than $50 for the whole project!
