Every human cell has 46 chromosomes in 23 pairs. The first 22 pairs are called Autosomes and the 23rd pair is called Allosomes. Allosomes determines the gender of a baby as female or male. The Y chromosome contains a gene called “SRY gene”, which are testes the determining factor. If SRY genes are expressed they convert the proto gonads into testes, which in turn produces testosterone and thus results in a baby boy. If SRY genes are absent (X chromosome) proto gonads are converted into ovaries and thus results in a baby girl.
Any mutation in SRY genes or its dysfunction may cause functional defects in the XY chromosome, which results in the formation of ovaries rather than testes in a male baby. Similarly, a mutation in CYP17 genes, NRC34 androgen receptor gene, XX male syndrome, Klinefelter Syndrome etc are the various genetic reasons for a transgender baby to form.
Apart from chromosomal defects, Physiological and Psychological factors also play a major role. Differences in the male and female brain cause a transgender to behave as their gender identified and gender expressed.
