*option to use HSAC Case Form No. 1 https://hsac.gov.ph/wp-content/uploads/HSAC-Case-Form-No.-1-REM-COMPLAINT.pdf but must be VERIFIED
- Original or certified true copies of documentary evidence supporting the cause of action

STEP 2: Pay the filing fees or submit a Certificate of Indigency

STEP 3: File the verified complaint

STEP 4: Wait for HSAC to process the verified complaint

STEP 5: Attend the mandatory mediation conference.

STEP 6: File position paper.

STEP 7: Wait for Judgment.
