Seated Leg Curl: Physio Exercise for Hamstring Strength & Knee Flexion Rehabilitation

Why seated rather than prone?
The seated position places the hip in a degree of flexion, which lengthens the hamstrings proximally before the knee flexion movement begins. This proximal lengthening increases the stretch placed on the muscle-tendon unit during the exercise, producing greater mechanical demand through the hamstrings compared to the prone variation. For patients managing proximal hamstring tendinopathy or hamstring strain rehabilitation, the seated position can be particularly effective when load tolerance allows. The seated format also reduces lumbar extension compensation, which is common in the prone leg curl.

Why isolated hamstring loading in rehab?
The hamstrings are among the most commonly injured muscles in athletic populations, and hamstring weakness is a contributing factor in anterior cruciate ligament loading, patellofemoral pain, and chronic low back presentations. Isolated loading through the Seated Leg Curl allows the hamstrings to be loaded directly and progressed systematically before they are asked to perform in complex multi-joint movements.

What to focus on:
Adjust the seat so the knee joint aligns with the machine's pivot point and the pad sits comfortably above the ankle
Begin with the leg at full extension and curl slowly through the full available range of knee flexion
Pause briefly at peak flexion before controlling the return phase
Lower the weight slowly — the eccentric phase is critical for hamstring adaptation and should not be rushed
Avoid allowing the hips to lift off the seat during the curl
Keep the movement smooth — do not use momentum to initiate the curl
This exercise is commonly prescribed for hamstring muscle strain rehabilitation, proximal hamstring tendinopathy, anterior cruciate ligament rehabilitation, patellofemoral pain syndrome with hamstring weakness, and post-operative knee rehabilitation where isolated posterior chain loading is indicated.
