All countries around the world are confronted by the challenge of making their healthcare systems sustainable. Such systems are squeezed between two irresistible forces, the escalating demands of ageing populations with multimorbid disease and the increasing costs of new technologies. Unless there is a change of approach the scandal of health inequality will only worsen; for the first time we are seeing a reversal in the increase in life expectancy in socially deprived regions of the UK.
