Anterior Drawer Special Test - Shoulder Exam
The Anterior Drawer test is a great special test for anterior shoulder laxity. I use it all the time to get a feel for how loose someone may be.
✅ The patient is supine. I like to stand at the head of the table.
✅ Hold the bicondylar axis of the elbow with your distal hand. This gives you complete control of the arm.
✅ Your proximal hand should grasp the humeral head between your thumb and first finger. This lets you really appreciate how much movement is occufing at the glenohumeral joint. I'm not a fan of using my whole hand on the humerus, I want to feel the humeral head.
✅ Glide the humeral head in an anteromedial direction. This is important! Follow the scapular plane of the glenoid, it's not straight plan anterior!
Believe it or not, despite wide use for decades. There's only one high quality study showing low sensitivity but high specificity.
I don't use this test to form a diagnosis, but rather just to get a general feel for anterior laxity.
