Double Relays: Purpose of & common mistakes

During a recent MLB game I was watching, a ball hit to deep right-center required a double-relay for a play at home plate. The throw to home was never made because the cutoff was botched. The announcers blamed it on a poor throw by the outfielder but in reality, the trailer on the double-relay team made two big mistakes. He was late getting out to the proper position and he was too far from the lead player.
I also received a question from a reader about double-relays and since I have not written about them in a while I thought I would create a video showing the basics and also the two most common mistakes made by the trailer on double-relays.
