Superior trunk block for awake shoulder surgery. Sala-Blach approach. Regional anesthesia Ultrasound

Ultrasound guided block of n. Suprascapularis, posterior divisjon and anterior division from upper trunk. We do the Sala-Blanch approach/ SPA approach.
The block is performed at the division of the the superior trunk, where it divides in n. suprascapularis, posterior division and anterior division. This approach is also called the SPA or Sala-Blach approach. We prefer to perform the block at this level and find it more easy to avoid direct contact with nerve tissue compared with Superior trunk. The more periferal, the more connective tissue the nerve contains.
This may play a role in how vulnerable the nerves are to injury.

With this teqnicue, the distance to phrenic nerve is increased.

Together with n. Supraclavicularis from cervical plexus this block works perfect for awake shoulder surgery in beach chair position.
