Axillary Block Decision Making Explained - Crash course with Dr. Hadzic

Axillary brachial plexus block is one of the most clinically applicable regional anesthesia procedures. While adherence to the standardized sequence of injections and dosing is important for its efficacy and safety, the variations in anatomy mandate modifications and clinical decision-making. Before ultrasound guidance, clinicians typically used a “trans-arterial approach”, where the location of the needle inside the axillary plexus sheath was confirmed by the appearance of arterial blood during needle advancement. By using the trans-arterial technique, ½ of the planned dose of local anesthetic would be injected on either side of the artery, as the needle is advanced “through-and-through” the artery.
With ultrasound guidance, we recommend the same technique, except that puncturing the artery as an internal landmark has been replaced by injections above and below the axillary artery while monitoring the spread of LA inside the axillary sheath. In the video, Dr Hadzic teaches a resident the process of decision-making and the sequence of an LA injection in a patient with axillary veins on the needle path.
