Creating Community: Reid's Story

Struggling with how to navigate life transitions for her middle-school aged son, Molly followed the advice of her county Developmental Disability coordinator to try a session of facilitated Person-Centered Planning. Getting everyone in Reid’s life on the same page—including his school, different family homes, friends and DDA Case Manager—became key to unlocking Reid’s potential for greater independence.

A Person-Centered Plan is a process of thinking and planning that builds on an individual's capacities and interests to help them live a full and meaningful life in their community. It is an evolving process of continual listening and learning, focusing on what is important to them now and in the future, and acting on this together with their family, friends and others. A PCP facilitator works with the individual and their support team to guide them toward a positive vision of their future, with clear steps to reach their goals.
