45-Degree Leg Press: Physio Exercise for Quad Strength & Lower Limb Rehab
At BeFit Training Physiotherapy in Edgecliff and Coogee, the 45-Degree Leg Press is used to develop quadriceps, gluteal, and hamstring strength through a bilateral lower limb pressing pattern — one of the most widely prescribed machine exercises in lower limb rehabilitation and progressive loading programs.
Why 45 degrees?
The angled platform positions the lower limb in a path that allows significant load to be moved through the knee and hip while reducing the spinal compressive forces associated with a vertical leg press. The 45-degree angle also accommodates a natural range of hip and knee flexion, making it accessible across a wide range of presentations and rehabilitation stages. Foot position on the platform can be adjusted to shift emphasis between the quadriceps, gluteals, and hamstrings — giving the clinician flexibility in targeting the appropriate muscle group.
Why machine-based pressing in rehab?
The 45-Degree Leg Press allows high load to be applied through the lower limb in a controlled, guided path — removing the balance, coordination, and trunk stability demands of free-weight squats. This makes it the appropriate tool in early-to-mid rehabilitation when the goal is to restore lower limb strength capacity before progressing to more complex movement patterns.
What to focus on:
Position the feet at shoulder width on the platform, with the angle of foot placement adjusted for the target muscle group
Lower the platform slowly through the full available range of knee flexion — aiming for at least 90 degrees where tolerated
Keep the knees tracking over the second and third toes throughout the descent
Press through the full foot to return to the start — avoid pushing exclusively through the toes or heels
Do not lock out the knees aggressively at the top — maintain a slight bend to keep tension on the working muscles
Keep the lower back flat against the seat throughout — avoid allowing it to curl off the pad at the bottom
This exercise is commonly prescribed for anterior cruciate ligament rehabilitation, patellofemoral pain syndrome, patellar tendinopathy, knee osteoarthritis, gluteal weakness, and post-operative lower limb rehabilitation following knee or hip surgery.
Exercise suitability depends on your individual assessment findings. Book an assessment at BeFit Training Physiotherapy, Edgecliff or Coogee, to determine the appropriate foot position, range, and load for your rehabilitation.
