Gynecomastia is abnormal hypertrophy of the male breast.1 It is estimated to be present in 65% of normal adolescents, with a greater incidence between the ages of 10 and 16. In addition to being unattractive, this enlargement limits activities for the patient, particularly during adolescence.

Among various classifications of gynecomastia, the one
by Simon et al,5 based on hypertrophic skin, is most useful:
Grade I. Small breasts without hypertrophic skin
Grade IIa. Moderate breasts without hypertrophic skin
Grade IIb. Moderate breasts with hypertrophic skin
Grade III. Large breasts with hypertrophic skin, similar to female breasts
There are many surgical procedures for correction of
gynecomastia.6-10 However, in the presence of Grade III
gynecomastia, characterized by a large volume of mammary and adipose tissue and excess skin, surgical options
are limited.
