Are you passionate about “What Matters to Me” — supporting people with health conditions to live well? An exciting opportunity has arisen within Keswick and Solway Integrated Care Community (ICC) from North Cumbria Integrated Care NHS Foundation Trust - for a Care Coordinator to join The Your Health Matters Team. We caught up with Jodie and Annarose to find out more about the team and how they’re helping people stay well in their communities 🎬

There are also opportunities for further training as an Assistant Practitioner and/or Nurse Associate for those looking to develop their career once established in the role.

You’ll work closely with GP practices and Primary Care Networks (PCNs) to make a real difference in people’s lives. 💙💚
