What's is retroperitoneal abcess-
Retroperitoneal abscess with subcutaneous extension: case report of a rare complication of percutaneous renal biopsy.

Why kidney Bx needed-

Lupus nephritis (also known as SLE nephritis) is an inflammation of the kidneys caused by systemic lupus erythematosus (SLE), an autoimmune disease. It is a type of glomerulonephritis in which the glomeruli become inflamed.

The diagnosis of lupus nephritis depends on blood tests, urinalysis, X-rays, ultrasound scans of the kidneys, and a kidney biopsy. On urinalysis, a nephritic picture is found and red blood cell casts, red blood cells and proteinuria is found. The World Health Organization has divided lupus nephritis into five stages based on the biopsy.
Infective complications following percutaneous renal biopsy are rare, even among immunocompromised. However it is important to be vigilant about such complications, to detect them early and prevent morbidity and mortality. We report a case of retroperitoneal abscess with extension to subcutaneous plane after a renal biopsy.

Case study-
Young girl presented with fever,malar rash,edema,joint pain,foamy urine since 2-3 months.Initially treated elsewhere. Consulted nephrologist else where for further investigation and was diagnosed with SLE, a kidney Bx was taken from left side and she was discharged with follow up antibiotics and other supportives. She was lost follow up and came to our nephrology dept with fever and pain and swealing left abdominal area.
She was not taken antibiotics after she had been discharged.
Ultrasound abdomen reveals a large retroperitoneal Abscess arising from left pelvic area extending to left subcostal margin with tenderness.
With the advise of nephrologist we planned for Evacuation of the huge abcess.
