How to Unlock the Lost in Rebirth

To unlock the Lost in Rebirth, you have to die in specific ways, in a specific order, with specific characters. You have to die in this exact order, and there must not be any other deaths in between:

1. Play as Isaac and die to a Mulliboom in The Basement or The Cellar
2. Play as Magdalene and die to your own bomb in the Caves or the Catacombs
3. Play as Judas and die to any Mom attack in the Depths or Necropolis. If you get killed by any other enemy during the Mom fight you have to start all over: it must be Mom who kills you.
4. Play as Azazel and die to any Satan attack in Sheol. If you get killed by the Fallen one of the Kamikaze Leeches that appears in the boss room you have to start all over: it must be Satan who kills you.

You can quit and restart in the middle of a run without losing your progress.

Do not pick up things like Lazarus' Rags, Judas' Shadow, or Ankh, because they will revive you as a different character when you die and will blow your progress. If you do pick up one of these items, just restart your run and you should be ok.

You actually can use seeds while you're doing this, except for the last run with Azazel. Here are the seeds I used. Shout out to The Binding of Isaac Wiki on GamePedia.com for these seeds.

1. Isaac - die to a Mulliboom in The Basement or The Cellar

PC: GPE3 2T1H (Must use Normal difficulty. You'll find 2 Mullibooms directly south of where you spawn. Go to the sacrifice room north of where you spawn to damage yourself enough that the Mulliboom will kill you)

2. Maggy - die to your own bomb in the Caves or the Catacombs

PC: ALFA 0NTS (Use Normal difficulty. Find Pyro in the first Treasure Room for bombs, right next to where you spawn)

3. Judas - die to any Mom attack in the Depths or Necropolis

PC: YR8E YXDV (Must use Normal difficulty. You will find Polyphemus in the first Treasure Room. Make sure you do NOT take any devil deals, because you will find Trinity Shield and the Sacred Heart in Angel Rooms. Keep the Book of Belial as your active item to make sure the Angel Rooms appear. The first boss, Duke of Flies, will drop Mom's Coin Purse. Pick up the orange pill for Health Up, and pick up the white pill for Full Health. Do not pick up the blue pills, because they are Speed Down.)

4. Azazel - die to any Satan attack in Sheol

You can't use seeds for Azazel.

How to Unlock the Lost in Afterbirth and Afterbirth+

There's a totally different way to unlock the Lost in the Afterbirth or Afterbirth+ DLCs. To unlock the Lost in Afterbirth and Afterbirth+ is die on the spikes in a sacrifice room while you're holding the Missing Poster trinket. You can be playing as any character when you do this.

You must use this new method, and you cannot use the Rebirth method with Afterbirth or Afterbirth+.

You could uninstall Afterbirth and Afterbirth+ and unlock the Lost the old way, but you'll likely lose some progress when you reinstall the DLCs.

There's some things you will need to have unlocked first, however. Here they are in order:

1. Unlock It Lives
2. Unlock the Dark Room
3. Unlock the Missing Poster

How to Unlock It Lives

Defeat Mom's Heart 11 times. Instead of fighting Mom's Heart, you fight It Lives at the end of the fourth chapter. After defeating It Lives, you can go down the stairs to Sheol or up the beam of light to the Cathedral.

How to Unlock the Dark Room

The Dark Room unlocks after you have defeated Satan 5 times. This will spawn the Negative each time after you have defeated Mom's Heart. Grab the Negative, go down into the Womb, beat It Lives at the end of the Womb's second floor, go down the staircase to Sheol, beat Satan, jump in the chest at the end of the level holding the Negative and you'll go to the Dark Room.
