Case 90: Manual of PCI - guidewire entrapment + fracture 2

PCI of an in-stent restenotic lesion of the mid right coronary artery was attempted. While trying to wire the vessel with a Runthrough guidewire, the wire became entrapped into the proximal RCA stent. Attempts to retrieve the guidewire by inserting a 2nd guide catheter and ballooning around the entrapped wire failed. Eventually the wire fractured, with a portion protruding into the aorta. The patient underwent emergent surgery with successful removal of the guidewire, followed by CABG.
