Dr. Ebraheim’s educational animated video about the Compartment syndrome describing the condition known as Volkmann's Ischemic Contracture or neglected compartment syndrome.
Volkmann's Contracture is the result of an untreated ischemic injury that causes muscle and nerve necrosis in the involved compartment.
Without the release of the muscle compartment, the muscle will die and the nerve will be injured. The limb will become deformed, dysfunctional and insensate which could necessitate amputation.
Most common etiologies:
•Neglected compartment syndrome: Early diagnosis and fasciotomy are necessary to avoid the development of Volkmann's ischemic contracture. Treatment is difficult.
•Crush syndrome
•Fractures associated with vascular injury
•Bleeding disorders
Most common forearm muscles affected:
•Flexor digitorum profundus is the single most commonly affected muscle.
•Flexor pollicis longus is the next most common
•Flexor digitorum superficialis
•Pronator teres
Clinical picture of the forearm
•Elbow flexion
•Forearm pronation
•Wrist flexion
•Thumb adduction
•IP flexion
•Severe palsy in the distribution of the median and ulnar nerves.
