Carlo Gambino recognized Fatico's capabilities, promoting him to the position of capo regime in 1957. During the 1960s, Fatico's crew, including figures like John Gotti, operated amidst the volatile atmosphere of East New York, where Puerto Rican gangs were prevalent. Adapting to the changing landscape, Fatico's crew began shifting their operations to Queens and the JFK airport industrial area.
In 1972, Fatico purchased the Bergin Hunt and Fish Club, which served as the crew's headquarters until John Gotti's incarceration in the 1990s.
