हमारे पहले वीडियो में हमने obstructive azoospermia के बारेमें जाना। आज इस वीडियो में Dr Jay Mehta , निल शुक्राणु के कारण ( Causes of Non-obstructive azoospermia | NIl Sperm Causes) के बारेमें बात करेंगे।

जैसे आपने हमारे पहले वीडियो में देखा है की Non obstructive azoospermia में testis में sperm का production कम रहता है। जैसे आप जानते है एक नार्मल इंसान तक़रीबन दिन के 1.5 - 2 करोड़ स्पर्म का निर्माण करता है पर Non Obstructive Azoospermia (NOA) में इंसान दिन के 10 हजार - 2 लाख तक स्पर्म का निर्माण करता है। इसमें पूरा tract नार्मल होता है मतलब vas deference और seminal vessicles सब नार्मल होते है पर कम स्पर्म production होने के कारण semen analysis में zero sperm count दिखाता है।

तो Non obstructive azoospermia के कारण क्या है ?
1. Idiopathic (unknown)
2. Hormonal - यह काफी आमतौर पे पाया जानेवाला कारण है और यह मोटापे (obesity ) के कारण हो सकता है और वजन घटाने से sperm count recover हो जाता है। इसके आलावा thyroid या brain के issues के कारण भी हो सकता है।
3. Chemotherapy / Radiation therapy के कारण भी Non obstructive azoospermia होता है।
4 . कभी कभी testis का position ठीक नहीं रहता (abdominal testis- Chryptorchidism)।
5. Genetic causes - जब Y chromosome में कुछ गड़बड़ होती है तब azoospermia होता है। इसमें Y chromosome microdeletion की जांच की जाती है।

इन सारे कारणों के वजहसे sperm production कम हो जाता है। जब sperm production कम होता है तब body brain को signal दे , देती है। Brain , शरीर के FSH hormone को बढ़ाके रखता है।
Non obstructive azoospermia में काफी बार testis examine करते है finger examination से , इससे काफी बार पता चलता है की testis size और volume दोनों कम है। यह दोनों Non obstructive azoospermia में काफी महत्वपूर्ण होते है। Non obstructive azoospermia में testosterone का value नार्मल भी हो सकता है या कम भी हो सकता है।
