100 Million Healthier Lives, convened by the Institute for Healthcare Improvement, brings together 900+ partners across 15+ countries who are pursuing integrated solutions to complex systems problems and achieving breakthrough outcomes. By developing solutions that approach mental, physical, social and spiritual well-being in an integrated way, we have proven that we can achieve breakthrough outcomes in health, well-being and equity. Members have achieved 90% improvements in hospitalization rates for children with asthma, 75% reduction in re-incarceration rates, and 50% improvement in chronic disease rates. With 100&Change funding, we would scale these solutions to achieve 100 million people living healthier lives by 2020 and transform the way the world thinks and acts to create health.
