Impact of Public Health Postnatal Home Visiting in NB on Breastfeeding among Disadvantaged Families

Breastfeeding is recommended to age 2 and beyond given known benefits for child health and development. Socioeconomically disadvantaged mothers may require additional supports, such as the Healthy Families Healthy Babies (HFHB) program. HFHB offers prenatal services that foster healthy pregnancy, and postnatal services that promote the healthy development of children from birth to age 2 years, provided by public health nurses and dietitians in NB to eligible first-time moms.

To examine the impacts of HFHB postnatal participation on breastfeeding.

A population-based retrospective outcome evaluation was undertaken at the NB Institute for Research, Data and Training (NB-IRDT) using linked individual-level administrative data. A birth cohort of babies born to first-time moms, between April 1, 2012, and March 31, 2014, was created. HFHB program data identified postnatal participants; and propensity scores (PS) identified a comparable group of non-participant first-time mom. Breastfeeding outcomes (duration and current) were assessed at 18-months using logistic regression models.

Nearly 5000 records were identified. The 1211 participants had an average age 24, 66% were single, and over 50% resided in areas with highest residential instability. Half of participants (49%) breastfed for less than a month, or not at all, while 10.1% were still breastfeeding at 18-months, as compared to 43.5% and 5.5% in PS-matches, and 32.7% and 6.6% in first-time moms, respectively. Participants were more than twice as likely to sustain breastfeeding at 18-months.

Postnatal home visiting programs have positive impacts on breastfeeding duration in at-risk populations. Results suggest the need for prenatal identification to enhance breastfeeding initiation.
