Improving population health on the frontline - a patient's view

Steve and Michele Bryan, a couple with type 2 diabetes, have seen great benefits to their health after professionals in the Berkshire West Integrated Care System used a Population Health Management (PHM) approach to understand their issues and offer them more personalised care options. Now, they have reduced their blood sugars and are better engaged with their self-care.
