Miner Configuration examples follow:

L3+ / L3++ PROHASHING CONFIG:

URL: prohashing.com:3333
WORKER: YourAccountName
PASSWORD: n=WorkerName a=scrypt

Nicehash Example:

URL: stratum+tcp://scrypt.usa.nicehash.com:3333
WORKER: AnyBitcoinAddressHere.WorkerName
PASSWORD: x

I didn't really cover unboxing/starting the miner, but seriously just open it up and plug in your ethernet cable, plug in all PCI-E power connectors and then plug in the psu, wait until you hear it take off like a jet (don't worry, you can't miss it!) and then follow the steps in the video to get setup.
