Peds NCLEX Cheat Sheet: The Tiny Human Danger List Peds questions are usually testing one thing: which kid is about to crash first? Stop memorizing every disease like you’re building a sad little flashcard museum. Look for patterns. Respiratory = biggest priority. RSV, asthma, croup, epiglottitis, pertussis. Red flags: retractions, nasal flaring, grunting, stridor, cyanosis, poor feeding, apnea, or a child getting quiet and sleepy from working too hard to breathe. Translation: quiet kid + bad breathing = bad day. Kids do not need to announce respiratory failure with a marching band. Dehydration/GI traps. Gastroenteritis, pyloric stenosis, intussusception, Hirschsprung, appendicitis. Watch for: dry mouth, no tears, low urine output, sunken fontanelle, tachycardia, delayed cap refill, lethargy. Projectile vomiting? Think pyloric stenosis. Currant jelly stool? Think intussusception. Rigid belly or worsening RLQ pain? Stop offering crackers and vibes. Infection/neuro red flags. Meningitis, measles, febrile seizures. Fever + stiff neck, photophobia, vomiting, confusion, seizure, or hard to wake = brain involvement until proven otherwise. Measles loves the classics: cough, coryza, conjunctivitis, Koplik spots, then rash. Infection control matters. If it’s in the stem, it matters. DKA. Vomiting, abdominal pain, dehydration, fruity breath, deep fast breathing, fatigue, confusion. That is not a stomach bug. That is the body running on battery acid because insulin left the group chat. Sickle cell. Pain crisis, fever, chest pain, shortness of breath, stroke symptoms, infection risk. Fever in sickle cell is not “watch and see.” Acute chest and stroke signs are emergency energy. CF and Kawasaki. Cystic fibrosis: thick mucus, recurrent lung infections, poor growth, salty skin, greasy bulky stools. Kawasaki: persistent fever, red eyes, rash, mouth changes, swollen hands/feet, coronary artery danger. Bottom line: Peds NCLEX is breathing, fluids, infection, perfusion, and neuro status. The diagnosis matters, but the crashing kid matters more. #nclex #nclexrn #nclexprep #peds #nursingstudent
