CRP to guide steroids in pneumonia #hospitalist #meded

Hospitalists — I’ve mostly ignored CRP in straightforward pneumonia.

This paper may change that.

In an individual-patient-data meta-analysis of randomized trials in hospitalized CAP, adjunctive corticosteroids reduced 30-day mortality overall.

But the more interesting signal was this:

Patients with CRP greater than 204 mg/L had a much larger mortality benefit — about 13% → 6%, NNT = 14.

Patients with CRP ≤204 did not show the same benefit.

And pneumonia severity score didn’t identify the benefit as clearly as CRP did.

Not 'steroids for everyone with CAP.'
But maybe: check the CRP when you’re wondering who might benefit.
