Counter-Strike - Why did Defuse Mode trump Hostage Mode?

I look back to the game's earliest days to investigate why- despite hostage mode coming first- defuse mode eventually reigned supreme. I also question how things might have been different, and ponder what would be required for the same change to happen today.
