V. COMPLEX CASE: Keratosis Obturans Removal - #356

This blocked ear wax removal extraction procedure is performed by UK pioneer and world-leading endoscopic ear wax removal specialist Mr Neel Raithatha (Consultant Audiologist) aka 'The Wax Whisperer’ at his clinic in Oadby, Leicester, UK.

Ear wax removal is necessary when ear wax build up becomes impacted. Symptoms causes by earwax blockage include: earache (otalgia), conductive hearing loss, blocked ear, tinnitus, itchiness and irritation, occlusion etc.

The client attended with right-sided Keratosis Obturans, which is an abnormal accumulation of desquamated dead skin that has failed to migrate naturally out of the ear canal. The dead skin can get infected, leading to erosion of the outer ear canal and surrounding ear structures. It is one of the most difficult things to remove from the ear canal since the plugs of dead skin are really compacted and adhered to the ear canal walls.

In this case, the client actually visited ENT a couple of days before, who had successfully managed to remove her left-sided Keratosis Obturans. However, they could not remove it from the right ear. She is due to undergo a CT scan to check to see if the Keratosis Obturans has affected/erodided her surrounding ear structures. Given the Keratosis Obturans was causing her significant distress, earache, occlusion and hearing loss, the client decided to visit me to see if I was able to remove for her.

Removing Keratosis Obturans, as explained earlier, is extremely difficult and time-consuming. It does not always make pretty viewing and requires a lot of patience. I had to use various instruments including the Zoellner suction probe, Fine End, St Barts Ear Hook and Jobson Horne to break up, loosen, tug and suction it out of the ear. Crocodile forceps were not appropriate to use as the dead skin was soft and would therefore just simply tear into little pieces as I was attempting to extract it.

The process of removing Keratosis Obturans can be quite uncomfortable for clients. This because of the skin adhesions to the ear canal wall and severe compaction of the dead skin deep inside the ear canal. Nonetheless, I was able to slowly, but surely, ensuring the client was comfortable at all times, able to remove the Keratosis Obturans from the right ear successfully. This provided the client with instant relief of her symptoms. She was beforehand concerned that it would not be possible to have it removed after her appointment with ENT.
