How Much Does Stamped Concrete Cost?

How much does stamped concrete cost? Generally stamped concrete cost $12.00 to $15.00 dollars per square foot.

What is included in that cost:
4" thick concrete that's 4000 psi mix design
Rebar or wire mesh reinforcement
Fibermesh reinforcement mixed in the concrete
Labor to install the forms
Labor to place and level the concrete
Labor to stamp the concrete
1 integral color for the concrete
1 release color for stamping the concrete
Sawed contraction / expansion joints
Cleaning and Sealing the concrete

Some things that may add to the cost of having stamped concrete installed are:
Using more than 1 stamp pattern
Having a Border stamped into the pattern
Having to do multiple placements
Needing a Pump truck (bad access to the project)
Using multiple colors or staining after the fact
