Types of Pelvic Ultrasound:
1. Transabdominal Ultrasound:
• Performed with the probe on the lower abdomen.
• Requires a full bladder to act as an acoustic window.
• Provides a broader view of pelvic organs.
2. Transvaginal Ultrasound (TVUS):
• Involves inserting a probe into the vagina.
• Offers a closer and more detailed view of pelvic structures.
• Typically performed with an empty bladder.
3. Doppler Ultrasound:
• Assesses blood flow in pelvic vessels.
• Used for evaluating ovarian torsion, tumors, or vascular abnormalities.
Indications for Pelvic Ultrasound in Females:
1. Evaluation of Uterus and Endometrium:
• Uterine fibroids
• Endometrial thickening (hyperplasia) or polyps
• Congenital uterine anomalies
2. Assessment of Ovaries and Fallopian Tubes:
• Ovarian cysts
• Polycystic ovary syndrome (PCOS)
• Ovarian torsion
• Tubo-ovarian abscess or ectopic pregnancy
3. Fertility Assessment:
• Monitoring follicular development
• Evaluation of endometrial lining during assisted reproduction
4. Abnormal Uterine Bleeding or Pain:
• Investigate causes like fibroids, adenomyosis, or malignancies.
• Evaluate pelvic pain or menstrual irregularities.
5. Pregnancy-related Conditions:
• Confirm early pregnancy or viability
• Evaluate for ectopic pregnancy
• Assess for miscarriage or retained products of conception
6. Pelvic Masses:
• Differentiation of benign versus malignant masses
• Aid in biopsy guidance if needed.
Findings on Pelvic Ultrasound:
1. Normal Uterus:
• Homogeneous myometrium.
• Endometrium changes depending on the menstrual cycle phase.
2. Uterine Abnormalities:
• Fibroids: Hypoechoic, well-circumscribed masses in the myometrium.
• Adenomyosis: Heterogeneous myometrium with small cystic spaces.
3. Ovarian Abnormalities:
• Follicles: Simple cystic structures (normal in reproductive-aged women).
• Ovarian cysts.
• PCOS: Multiple small follicles in a “string of pearls” pattern.
4. Pathological Conditions:
• Hydrosalpinx:
• Pelvic inflammatory disease:
Doppler Ultrasound for Ovaries:
• Evaluates blood flow to distinguish between benign and malignant masses.
