Here we have a patient with a positive bear hug test indicative of a subscapularis tear in the shoulder. The subscapularis muscle is a powerful internal rotator of the shoulder and provides a lot of stability at the gleno-humeral joint.
👉The patient holds the hand of the affected shoulder up against the chest on the opposite side and tries to hold it there while the therapist tries to pull it away. Inability to hold the hand up against the chest is a sign of an injury to the subscapularis muscle.
