CAT 3406E white smoke + hard idle + shaking + vibration

Was driving on freeway and felt some unusual vibrations coming from the steering wheel and seemed like it was coming from the engine.

Pulled over to inspect and found the engine to be idling extremely rough with a lot of vibration and lots of white smoke coming from the blow by hose.
