Mid-air Collision Compilation #2 - Besiege

Piedmont 22 was a commercial passenger flight from Asheville, North Carolina to Roanoke, Virginia on July 19th, 1967 carrying 79 people.

Crash:

Not long after take-off, the plane struck a Cessna carrying 3 people completely destroying the 727, everyone on board both planes died.

Investigation:

The controller hadn't prevented the 2 planes from colliding because he was inattentive, there was also a fire started in the cockpit ashtray of the 727 causing panic and a lack of situational awareness.
The Charkhi-Dadri Mid-air Collision involved 2 planes, Saudia 763 and Kazakhstan 1907 on November 12th, 1996.

Saudia 763 was a commercial passenger flight from Delhi, India to Dhahran, Saudi Arabia carrying 312 people.

Kazakhstan 1907 was a commercial passenger flight from Chimkent, Kazakhstan to Delhi, India carrying 37 people.

Crash:

While the Kazakhstan plane was landing and the Saudia plane just took off both aircraft collided at about 14,000 feet, the vertical stabilizer and right wing of the Kazakhstan plane sliced through the left wing and tail of the Saudia plane putting the Saudia plane into an uncontrollable dive crashing and killing everyone on board, this also caused the Kazakhstan plane to make an uncontrolled descent crashing into a field also killing everyone on board, there were 349 fatalities in total and is the 3rd deadliest aviation accident ever and the most deadly Mid-air collision in history.

Investigation:

The Saudia plane was supposed to stay at 14,000 feet while the Kazakhstan plane was supposed to stay at 15,000 feet, the accident was mostly due to the Kazakhstani pilot's failure to follow the instructions of the ATC and Flight Engineer, it wasn't all their fault as there was a pretty big language barrier, you had 3 Kazakhstani pilots talking to an Indian ATC in English which isn't the easiest way to communicate. This eventually leads to the Kazakhstan plane descending to 14,000 feet, the Flight Engineer noticed and tried to get the captains to pull up which they did but it was already too late and the planes collided. The ATC's monitor didn't display the speed and altitude of the aircraft which also contributed to the accident.
The 1976 Zagreb Mid-air Collision involved 2 planes, British Airways 476 and Inex-Adria Aviopromet 550 on September 10th, 1976.

British Airways 476 was a commercial passenger flight from London, England to Istanbul, Turkey carrying 63 people.

Inex-Adria Aviopromet was a commercial passenger flight from Split, Yugoslavia to Cologne, West Germany carrying 113 people.

Crash:

At 10:14:41 AM both planes collided over Zagreb, Yugoslavia, the left wing of the Inex-Adria plane sliced through the cockpit of the British Airways plane, the Britsh Airways plane Disintegrated from the explosive decompression and the Inex-Adria plane went into a steep dive and crashed. Everyone on board both planes died with 176 fatalities in total, it is also British Airways only fatal accident.

Investigation:

The Inex-Adria Dc-9 had requested to climb from 31,000 feet to 35,000 feet which were granted, they were given a new squawk code by the ATC so another ATC could contact them, however, the Dc-9 didn't contact the new ATC immediately, they also began climbing prematurely and were already at 32,500 feet when they finally contacted the new ATC. The old ATC saw what was happening as British Airways 476 crossed paths with the Inex-Adria plane and was at 33,000 feet, the ATC contacted the Inex-Adria plane immediately and told them to stop climbing.

1. The ATC told them to do this in his native Serbo-Croatian language so the British Airways plane didn't know what was happening.

2. When the ATC told the Inex-Adria plane to stop ascending it was already at 33,000 feet sealing the fate of both the planes. If he had done nothing then this would've been a near miss.
TWA 554 was a commercial passenger flight from Pittsburgh, Pennsylvania to Dayton, Ohio on March 9th, 1967 carrying 25 people.

Crash:

The Dc-9 crashed into a privately owned Beechcraft baron with its 1 pilot killing him instantly, the Dc-9 broke up and crashed near Urbana, Ohio killing everyone on board.

Investigation:

Both planes were flying under VFR (Visual Flight Rules) which means both planes had to see and avoid each other, the Beechcraft was difficult to spot on the ATC's monitor and due to blindspots and very low visibility from the haze and clouds at the time, it was almost impossible for either plane to spot one another.
