ยาต้านการแข็งตัวของเลือด…โดยมากแพทย์จะสั่งให้กับผู้ป่วยที่มีแนวโน้มที่จะเกิดก้อนเลือดหรือลิ่มเลือด เช่นผู้ป่วยที่มีอาการของภาวะหัวใจเต้นผิดจังหวะ ผู้ป่วยที่มีประวัติมีภาวะสมองขาดเลือดชั่วคราว และผู้ป่วยที่ไม่สามารถเคลื่อนไหวได้หรือผู้ป่วยติดเตียง…ในที่นี้ผมจะเน้นไปที่ผู้ป่วยติดเตียงและผู้ป่วยที่ไม่สามารถเคลื่อนไหวได้ ซึ่งมีแนวโน้มที่จะเกิดลิ่มเลือดนะครับ มันเป็นความรับผิดชอบของแพทย์ในการที่จะตรวจเช็คปริมาณการให้ยาต้านการแข็งตัวของเลือดกับผู้ป่วยอย่างสม่ำเสมอนะครับ เช่นผู้ป่วยที่ได้รับยาวาร์ฟาริน (Warfarin) แพทย์จะต้องตรวจวัดค่าไอเอ็นอาร์ (INR) ทุก2-4 สัปดาห์ เพื่อปรับเปลี่ยนขนาดการให้ยาให้เหมาะสม เพราะถ้าหากปริมาณยาน้อยจนเกินไป ผู้ป่วยก็มีแนวโน้มที่จะเกิดลิ่มเลือดได้ เช่นกันนะครับ ถ้าให้ยาปริมาณมากเกินไป ผู้ป่วยก็เสี่ยงที่จะมีเลือดออกหรือมีภาวะสมองขาดเลือดได้…ผมอยากที่จะกระตุ้นเตือนให้แพทย์ด้วยกัน ช่วยกรุณาเอาใจใส่กวดขัน ดูแล และแสดงความเห็นอกเห็นใจต่อผู้ป่วยที่ไม่สามารถเคลื่อนไหวได้ หรือผู้ป่วยติดเตียงเป็นพิเศษหน่อยนะครับ! ด้วยความปรารถนาดี
Blood thinners are often prescribed to patients who are prone to developing blood clot. Such as patients with heartbeat irregularities, patient with history of transient ischemic attack and immobile/bedbound patients. Here I want to concentrate on bedbound and immobile patients as immobile patients are prone to develop clots. It is the doctor’s responsibility to regularly check the patient’s anticoagulation medication’s dosage. So, let’s say the patient is on Warfarin, the doctor must check the patient’s INR level every 2-4 weeks to adjust the medication accordingly. If the medication is too low of a dose, the patient may develop blood clot, if the medication is too high of a dose, then the patient runs the risk of bleeding and even stroke. I want to urge fellow doctors to please be meticulous and take the best care and show compassion towards immobile or bedbound patients. Best Wishes
