STPA Hazard Analysis perspective: Flight Control System (FCS) believed the aircraft was on the ground, when it wasn't,... and so it provided (Unsafe) Control Actions (UCAs) and boom $196.5M+ up in smoke.

But... it was a bit more complicated than that...
